What to bring for your appointment:
- Completed forms
- Your health insurance card
- Photo ID
- All diagnostic test-X-ray, MRIs, CT Scans, EMG
- Medical records from previous treating physicians
- A list of current medications
- Co-payments
All minors under 18 years of age must be accompanied by a parent or legal guardian in order to be treated by our physicians. All new patients are required to arrive 20 minutes prior to their appointment time. |
